I won 1800 on a slot machine last April and never recieved a W-2G from the casino. I did get a W-2g report from them, but it does not have the Federal ID number on it. Does anyone know the Federal ID number for Choctaw Casinos in Durant Oklahoma? Do I have to fill this out on the tax form?
